dotCover coverage button not appearing

While the dotCover menu appears and lets be run coverage analysis for applications, the coverage button shown is demonstration videos of dotCover to let me run coverage analysis of unit tests does not.  Thus, the only way for me to get coverage of, say, an nUnit test suite now is to run the external nUnit runner (GUI or command-line) and then coverage child processes, which is terribly awkward.

I have attempted a reinstall of ReSharper and dotCover, to no effect.  I have double-checked the preferences of both products.

Any pointers?

This is for teaching the course "Analysis, Design, and Software Architecture" at the IT University of Copenhagen.

The students' current assignment focuses on synthesizing hand written unit tests in nUnit and automatically generated unit tests in MSTest (via MSR's PEX) and measuring the overall test coverage.  Without dotCover working properly, this is a complex proposition.  

The students love your products by the way, so I expect that your free academic licensing here will result in many sales down the road.
